Questions to take along
Take your questions into the visit.
Keep the answers together. A clear name, a clear bill and a clear follow-up plan make the next decision easier.
Get the name, the bill and the next step.
Bring this sheet to an in-person visit or keep it beside the screen. Mark a box when the answer is clear.
Questions for your care conversation.
It is fine to ask for time.
Ask for a written estimate and a copy of the care plan. Read them when you have space to think.
If a term is unclear, ask for it in ordinary words. Your questions do not need to sound medical.
Bring the context with the questions.
- The health concern you want to discuss.
- A list of medicines and other products you use.
- Any past test results you already have.
- Your plan name and the answers it has given you.
